Qualifications

• Active unencumbered NP license required.
• ANCC or AANP board certification required.
• Vaccination and preventive testing as per policy prior to hire.
• Knowledge of CMS/Medicaid and Health Plan benefit structures is beneficial.

Responsibilities

• Conduct comprehensive home assessments or telehealth visits.
• Document encounter findings and care plans in the EHR.
• Coordinate with PCP and health plan for follow-up and additional services.
• Identify and address gaps in care and chronic condition management.
• Maintain HIPAA compliance and patient confidentiality.
