Job summary
ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem remote work covering Klamath County, OR. The provider will conduct comprehensive in-home and telehealth assessments to evaluate health status, risks, and care needs, aiming to keep members out of hospital.
The role requires ANCC/AANP-certified NP with an active license, CPR, and HIPAA compliance. In‑home visits and telehealth assessments are compensated with a pay range and a 3.5k sign-on bonus; CCS offers a 401(k) with 3%
Qualifications
- Active unencumbered nursing license
- ANCC or AANP family/adult/gerontology/EM certification
- CPR certification
- Vaccination/testing compliance before hire
- Home health experience a plus
Responsibilities
- Review assigned member population comprehensively and complete documentation
- Document active medical conditions per CMS/DHHS and client requirements
- Communicate results and care recommendations to PCP/health plan
- Provide information to improve members’ knowledge about conditions
- Close gaps in care and escalate emergent situations per policy
- Perform and document point-of-care testing
- Maintain HIPAA compliance and company policies
- Be available for ~37 hours/month (25 slots)
