Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP Nurse Practitioner certification in Family, Adult, Gerontology or Emergency Medicine.
• Current CPR certification.
• Vaccination and preventive testing compliance before hire (Hep B, Flu, MMR, Tdap, Varicella, TB).
• Ability to practice autonomously in a remote setting and coordinate with PCP/health plans.

Responsibilities

• Perform comprehensive member reviews and complete encounter documentation.
• Document active medical conditions per CMS/DHHS policies and client requirements.
• Communicate assessment findings, recommendations and follow-up timelines to PCP and health plans.
• Close care gaps and provide evidence-based treatment plans for chronic conditions.
• Conduct Point of Care (POC) testing and document results.
