Job summary

UnitedHealth Group, through Optum HouseCalls, is expanding its in-home assessment program for Medicare Advantage members. The role focuses on comprehensive, non-prescribing field work delivered in patients’ homes with a strong emphasis on education, care coordination, and outcome improvement.

The Advanced Practice Clinician will perform annual home visits, coordinate with PCPs, and work in an interdisciplinary team to close gaps in care while addressing social determinants of health.

Qualifications

• Active NP license or eligibility by start date.
• National certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care (or ability to obtain).
• Active and unrestricted driver's license with access to reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• If allowed to practice independently, obtain required approvals within set timelines.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
