Job summary

UnitedHealth Group’s Optum HouseCalls program seeks an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plans. The role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Licensure must be active in the practicing state, with flexible 8am–8pm scheduling and no on-call, weekends, or holidays. The ideal candidate holds NP/PA certification, current state licensure, and experience with electronic medical

Qualifications

• For NP: Active ANCC or AANP Family/Adult/Geriatric licensure.
• For PA: NCCPA certification and state licensure.
• Active, unrestricted licensure in practicing state with no disciplinary actions.

Responsibilities

• Conduct comprehensive in-home assessments and vitals review.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Educate patients on diseases, medications and adherence.
• Address social determinants of health and provide referrals as needed.
