Job summary

UnitedHealth Group, through Optum Home & Community Care, seeks Advanced Practice Clinicians for in-home annual health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and PCP collaboration to improve outcomes.

The position offers flexible scheduling and a path to growth within the Optum HouseCalls team, with focus on community-based care delivery.

Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state with no disciplinary actions.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including history, vitals, and exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
