Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments. You will review health history, medications, and risk factors, order tests, and coordinate with physicians and case managers. Services are delivered at the member’s home or SNF under Matrix guidelines.

The role offers full-time opportunities with a focus on flexible scheduling, travel to members’ residences, and ongoing education and support to advance clinical practice.

Qualifications

• Master’s degree in nursing or commensurate experience and NP licensure completed.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Family or Gerontology specialties per listed options.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance coverage for the state of residence.

Responsibilities

• Conduct adult/geriatric health assessments, including medical history, diagnosis, treatment, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers and other staff; ensure clear documentation.
• Provide services at members’ residences, including homes or SNFs.
