Job summary

Matrix Medical Network provides in-home comprehensive health assessments across the United States. Providers perform geriatric health evaluations, review medical history, medications, and social factors to close care gaps while collaborating with PCPs and care teams.

Services are delivered at the member’s home or skilled facilities, with emphasis on quality outcomes, education, and follow-up. A flexible schedule, competitive pay, and ongoing professional development support are offered.

Qualifications

• Master’s Degree in Nursing and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in an Adult/Family/Gerontology specialty.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license with adequate automobile insurance.
• Strong computer skills and familiarity with health/medical record software.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Coordinate with PCP on education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
