Job summary

UnitedHealth Group through Optum Home & Community Care seeks a HouseCalls Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and coordination with primary care providers.

Flexible scheduling with 8am–8pm coverage across seven days, no on-call, weekends, or holidays. Travel to patient homes with reliable transportation and a competitive compensation package with base pay

Qualifications

• Active NP or PA certification for family/adult/geriatric primary care.
• Current state licensure without disciplinary actions.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Perform comprehensive in-home health assessments for Medicare Advantage members.
• Identify diagnoses for care management and treatment planning; document findings for PCPs.
• Educate patients on diseases, medications and adherence; address social determinants of health.
