Job summary

Qualifications

• Active ANCC or AANP certification for NPs or NCCPA for PAs.
• Active and unrestricted state licensure for practice.
• Driver's license with reliable transportation for home visits.
• Ability to travel to patient homes and navigate varied environments.
• Eligible/licensing requirements must be met by start date.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history and vitals.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent situations and intervene as needed.
• Educate patients on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
