Optum HouseCalls is hiring an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role focuses on improving outcomes through education, gap closure, and collaboration with PCPs.

Requirements include NP/PA certification, unrestricted licensure, and a valid driver’s license with reliable transportation for home visits. Competitive compensation and benefits are offered.

Competenze

• Active and unrestricted licensure in the practice state.
• Current, unrestricted driver's license and reliable transportation.
• Certification in family/adult/geriatric primary care (NP) or NCCPA for PA.

Mansioni

• Conduct in-home health assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Coordinate with PCPs to close care gaps.
• Educate patients on diseases, medications, and adherence.
• Identify and address social determinants of health and provide referrals.
