Job summary

UnitedHealth Group seeks a HouseCalls Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage and other members. You will educate patients, close care gaps, and collaborate with PCPs to improve outcomes in a field-based setting.

This role emphasizes flexible scheduling, 24–40 hours weekly, and non-presence of on-call, weekends or holidays, with a strong focus on delivering patient-centered, accessible care.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted licensure to practice in state with no disciplinary actions
• Active and unrestricted driver's license and reliable transportation

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on disease processes, medications and adherence
• Address social determinants of health and provide referrals as needed
