Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Visits last up to one hour and cover health history, medications, adherence, and risk factors to identify care gaps and improve outcomes.

Applicants should hold NP licensure in their practice state, board certification, and current BLS/ACLS. Driving license required; we support licensure expansion and offer flexible FT opportunities, mileage

Qualifications

• Master’s degree in nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in relevant NP specialization.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric health assessments including medical history and treatment plans.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCP, case managers, social workers and family members.
• Provide care at member residences (home or SNF).
• Document findings and follow up per Matrix guidelines.
