Matrix Medical Network is seeking qualified Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or SNFs. Each visit lasts up to an hour and includes review of medical history, medications, and risk factors, with documentation per Matrix guidelines.

Requires NP licensure in your state, BLS/ACLS, and at least 1 year of NP experience. Flexible scheduling and potential FT opportunities accompany competitive pay and benefits.

Qualifications

• Must hold NP licensure in state of practice including prescriptive authority if needed.
• BLS, ACLS or CPR certification required.
• Minimum 1 year NP experience; new grads encouraged to apply.
• Valid state driver''s license and auto insurance for residence state.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsabilités

• Perform adult/geriatric health assessments including history, diagnoses, education, referrals.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and caregivers.
• Provide services at member''s residence including homes or SNFs.
