Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or SNFs. Assessments cover medical history, diagnosis, treatment, and health education, with clear documentation per Matrix standards.

You will order tests as needed, collaborate with PCPs and care teams, and provide patient-centered follow-up. A valid NP license, prescriptive authority, and BLS/ACLS/CPR are required, with driving and insurance for residence state preferred.

Qualifications

• Master’s degree or commensurate NP licensure.
• Current NP licensure with prescriptive authority.
• Board certification in Adult/Adult Gerontology Primary Care, Family, Emergency or Acute Care.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license; auto insurance coverage for residence state.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare staff as appropriate.
• Provide services at the member’s home or Skilled Nursing Facilities as required.
• Coordinate education and follow-up with Primary Care Physician (PCP).
