Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments. Each visit lasts up to one hour and covers medical history, medications, and social factors to close care gaps. The role collaborates with physicians, case managers and families to improve health outcomes.

Qualified candidates hold NP licensure with prescriptive authority, board certification, and current BLS/ACLS. A valid driver’s license and experience with EMR systems are preferred.

Qualifications

• Must have NP licensure and prescriptive authority in the practice state.
• Board certification in Adult, Gerontology, Family or related NP specialties.
• Current BLS/ACLS or CPR certification is required.
• Minimum 1 year NP experience; new grads encouraged to apply.
• Valid state driver’s license and auto insurance as required.
• Strong computer skills and experience with health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Perform adult/geriatric health assessments in members’ homes.
• Review medical history, medications and social risks during CHA.
• Coordinate with PCPs, case managers and family members.
• Administer point-of-care tests and lab orders as needed.
• Document findings clearly per Matrix guidelines.
