Job summary

Matrix Medical Network is seeking skilled Nurse Practitioners to perform comprehensive in-home assessments. Providers review health histories, medications, and social factors to identify gaps and improve outcomes during visits that may last up to an hour.

Responsibilities include ordering tests, coordinating with PCPs, and delivering care in patients' homes or SNFs. Applicants should have NP licensure, board certification, and a valid driver’s license, with flexible scheduling and regional

Qualifications

  • Master’s degree in nursing or commensurate experience with NP licensure.
  • Current NP licensure in the practicing state with prescriptive authority as needed.
  • Board certification in relevant adult/gerontology/family specialties is required.
  • Active BLS/ACLS/CPR certification and clean driving record are mandatory.

Responsibilities

  • Conduct adult/geriatric assessments with medical history and documentation.
  • Administer and order point-of-care and lab-based testing as needed.
  • Collaborate with physicians, case managers, social workers, and caregivers.
  • Provide services at patients’ homes or Skilled Nursing Facilities as appropriate.
