Job summary

Amedisys is seeking a Registered Nurse for PRN Day shifts to support home health patients in their homes. You will perform assessments and coordinate care with the care team to ensure consistent, quality patient outcomes.

Responsibilities include developing, implementing, and updating individualized care plans, monitoring patient progress, and educating families. Requirements include an active RN license, CPR, and a valid driver's license, with on-call duties as needed.

Qualifications

• Current RN license in the applicable state(s).
• CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care and maintains continuity of patient care.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Educates families on patient condition, needs and care.
• Performs on-call responsibilities as assigned.
