ChenMed is seeking a Nurse Practitioner to join our clinical operations team, delivering direct patient care across ChenMed centers, SNFs, and home settings as assigned. You will conduct geriatric assessments, take medical histories, perform exams, and develop care plans with clear documentation.

You will collaborate with physicians and a multidisciplinary team, ensure guidelines are followed, and participate in rounds and quality initiatives.

Qualifications

• Requires NP license and 2+ years clinical NP experience.
• Experience in Family Medicine, Internal Medicine or geriatrics preferred.
• Strong communication and bedside manner essential.
• Ability to work independently and collaboratively.

Responsabilités

• Independently assesses acute and non-acute clinical problems and documents plans.
• Plans care based on patient population and protocol, considers culture and support systems.
• Writes admission, transfer and discharge orders.
• Orders and interprets lab and diagnostic studies.
