Qualifications

• Licensed/Board Certified/Board Eligible in internal or family medicine.
• Direct patient care in ambulatory health care centers.
• Ability to perform comprehensive assessments and develop plans of care.
• Strong documentation and communication with care team.

Responsabilités

• Independently assesses acute and non-acute clinical problems.
• Documents progress notes and summaries per ChenMed standards.
• Collaborates with multidisciplinary team to optimize patient care.
• Participates in clinical rounds and meetings to discuss patient management.
• Initiates discharge planning and coordinates transitions to appropriate care levels.
