Job summary

ChenMed is seeking a Nurse Practitioner to join our clinical operations team in Milwaukee, WI. The NP will provide direct patient care across ChenMed medical centers, SNFs and home settings, delivering high-quality, patient-centered care for geriatric and chronic disease management.

You will assess, diagnose, treat, educate, and coordinate care with physicians and a multidisciplinary team, maintain documentation, and participate in rounds and quality initiatives.

Qualifications

• ARNP or similar advanced degree in Nursing required.
• Current Nurse Practitioner Certification in the State required.
• Board certification by AANP or ANCC preferred.
• Current DEA number may be required depending on state.
• Minimum 2 years of NP clinical experience in Family Medicine, Internal Medicine, or Geriatrics.
• BLS certification from AHA or Red Cross required within first 90 days.

Responsibilities

• Provide direct patient care in ChenMed centers, SNF and home settings.
• Perform comprehensive assessments, diagnose, treat, educate and develop care plans.
• Order and interpret labs and diagnostics; coordinate referrals.
• Document progress notes, participate in rounds and team meetings.
• Collaborate with multidisciplinary team to achieve patient care goals.
• Prescribe medications within state practice and ensure patient safety.
