ChenMed is seeking a Nurse Practitioner to join our clinical operations across ChenMed centers, SNFs, and home settings. You will perform comprehensive assessments, develop care plans, and coordinate with physicians and the multidisciplinary team to ensure high-quality patient care.

The role requires NP licensure with active certification, 2+ years in family medicine or geriatrics, and 50–75% local travel. You will operate under ChenMed standards for quality, service, and teamwork, contributing

Qualificações

• ARNP or similar advanced degree required.
• Current Nurse Practitioner Certification in the state of practice required.
• Board certification by AANP or ANCC preferred but may be required in some states.
• Current DEA number from the DEA may be required depending on state.
• Minimum 2 years NP clinical experience in Family/Internal Medicine or Geriatrics with autonomy.
• BLS certification from AHA or Red Cross required within first 90 days.

Responsabilidades

• Independently assesses acute and non-acute clinical problems and documents history and plans.
• Plans patient care considering cultural background, level of understanding and support systems.
• Writes orders for admissions, transfers, discharges and medications.
• Refers to specialists when indicated and documents progress notes and summaries.
• May perform invasive procedures after competency is documented.
• Participates in rounds and collaborates with the care team to meet patient needs.
• Initiates life-saving procedures when needed and communicates management strategies.
