Job summary

ChenMed LLC is seeking a Nurse Practitioner to deliver direct patient care across ChenMed centers, SNFs, and home settings. You’ll perform assessments, develop plans of care, educate patients and coordinate with a multidisciplinary team.

Required are NP certification in the practicing state, 2+ years in Family Medicine/Geriatrics, and BLS. You may travel locally and participate in rounds, documentation, and quality initiatives.

Qualifications

• ARNP or similar advanced degree in Nursing required.
• Current NP certification in the practicing state.
• Board certification preferred but may be state dependent.
• Minimum 2 years of clinical NP experience in Family/Internal medicine or geriatrics.
• BLS certification within 90 days of employment.

Responsibilities

• Assess acute and non-acute clinical problems independently.
• Perform physicals, histories, develop management plans and educate patients.
• Order tests and medications, refer to specialists when needed, document thoroughly.
• Collaborate with physicians and care team to ensure quality care.
• Participate in rounds, meetings, and risk/quality management programs.
