Job summary

ChenMed LLC seeks a Nurse Practitioner to provide direct patient care across ChenMed centers, SNF and home settings. You will conduct geriatric assessments, develop nursing care plans, document progress, and refer to specialists as needed.

You will advocate for patients, coordinate with the multidisciplinary team, and may perform invasive procedures after competency. Extensive travel may be required to meet patient needs.

Qualifications

• Geriatric assessment including history, exam, diagnosis and treatment planning.
• Develop and document nursing plan of care and patient education.
• Independent assessment of acute and non-acute clinical problems.
• Maintain thorough progress notes and summaries.
• Refer to specialists as indicated and coordinate care.
• Create and document patient management plans with patient-specific factors.
• Advocate for patient and family in care decisions.
• Prepare admission, transfer and discharge orders.
• Communicate management strategies to the care team.
• May perform invasive procedures after approved competency.
• Willingness to collaborate with physicians, nurses and support staff.

Responsibilities

• Perform geriatric assessments and develop comprehensive care plans with timely documentation.
• Lead rounds, provide patient education and coordinate referrals.
• Manage patient care plans and track clinical outcomes with attention to safety.
