Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments for geriatric and at-risk members. You’ll review medical history, medications, social factors, and provide care recommendations during visits at members’ homes or SNFs.

The role requires active NP licensure, board certification, and flexibility to travel extensively (80–90%). Strong communication and EMR proficiency are essential for documentation and coordination with care teams.

Qualifications

• Master’s Degree in Nursing or NP equivalent.
• Current NP licensure in state of practice.
• Board certification in Adult/Family/Gerontology specialties.
• BLS, ACLS certification.
• 1 year NP experience.
• Willingness to travel up to 80-90% including overnight travel.
• Valid driver’s license and auto insurance.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
