CareerNerds, LLC seeks a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care in the Greater Sacramento area. This role offers relocation assistance and a Monday–Friday 8–5 schedule or a 4-day, 10-hour shift option.

The successful candidate will hold an active California NP/PN license, NP board certification, DEA and BLS certifications, and be comfortable treating underserved populations with EPIC experience, strong teamwork, and

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years of clinical experience preferred, particularly in community health or underserved populations.
• Strong commitment to serving low-income, homeless, or medically underserved communities.
• Comfortable treating three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred.

Responsabilités

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans in collaboration with the Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling services.
• Maintain accurate and timely documentation in accordance with agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care.
