Job summary

CMR Recruiting is seeking a Nurse Practitioner for a Skilled Nursing Facility in Ohio. This full-time, in-person role runs M-F 9-5 with competitive pay and comprehensive benefits.

Must hold an Ohio NP license with at least 2 years of NP experience; SNF experience preferred. You will perform admissions exams, manage chronic conditions and medications, coordinate transitional care, and engage with families to support optimal outcomes.

We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Kettering, OH for bedside patient care. This is a full-time, in-person position M-F 9-5.

Qualifications of the Nurse Practitioner ( NP ):

• Valid Nurse Practitioner ( NP ) license in the state of Ohio
• 2+ years of experience as a Nurse Practitioner ( NP )
• Experience working in a Skilled Nursing Facility ( SNF ) preferred

Responsibilities of the Nurse Practitioner ( NP ):

• Upon admission to the facility, examine residents and their medical records.
• Check ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
