Qualifications

• RN licensure with MN Board of Nursing.
• 1 year Long Term Care experience.
• Basic Life Support (BLS) certification.
• CPR & AED certifications as required.

Job Overview

Lake City Care Center is nestled near the picturesque Lake Pepin in southeast Minnesota. Our skilled nursing facility can provide care for up to 90 residents across our Transitional Care and Long-term care units, supported by a dedicated team committed to Ebenezer’s mission of fostering longer, healthier, and more meaningful lives through healing, discovery, and education. Nursing schedule includes;

• 80 hours every two weeks
• Day shift; 8 hour shift
• *NO weekend rotation

Position Summary

Provide leadership and oversight to nursing staff to ensure high-quality resident care, regulatory compliance, and continuity of services. This role is responsible for coordinating clinical operations, supervising nursing personnel, supporting quality improvement initiatives, and serving as a key resource for residents, families, physicians, and staff.

Key Responsibilities

Clinical Leadership & Resident Care

• Oversee daily nursing operations and ensure care is delivered according to individualized care plans.
• Supervise, direct, and delegate nursing responsibilities while ensuring compliance with scope of practice standards.
• Monitor quality of care through rounds, chart audits, and observation of staff performance.
• Ensure medications, treatments, and physician orders are carried out accurately and timely.
• Coordinate admissions, transfers, discharges, and resident care planning processes.
• Participate in interdisciplinary team meetings, care conferences, and quality assurance activities.
• Ensure resident records, care plans, code status documentation, and POLST forms are accurate and up to date.
• Communicate changes in resident condition and care plans with physicians, residents, and families.

Staff Leadership & Development

• Provide coaching, education, and ongoing support to nursing staff.
• Conduct performance evaluations and address performance concerns promptly.
• Participate in employee disciplinary and corrective action processes in collaboration with Nursing and HR leadership.
• Coordinate nursing team meetings and promote a positive, collaborative work environment.
• Serve as a role model and mentor for staff.

Compliance & Quality Assurance

• Ensure compliance with state and federal regulations, nursing standards of practice, and organizational policies.
• Investigate and follow up on incidents, medication errors, and vulnerable adult reports.
• Participate in audits, QAPI initiatives, and continuous quality improvement projects.
• Identify operational concerns and recommend solutions to leadership.

Operations & Administrative Support

• Ensure appropriate staffing, equipment availability, room assignments, and infection control practices.
• Monitor mealtime support, resident services, and overall care delivery throughout the facility.
• Serve as the facility supervisor and emergency response leader as assigned.
• Assume administrative responsibilities in the absence of the Administrator or Director of Nursing when designated.
• Conduct tours for prospective residents and families as needed.li>
• Participate in scheduled weekend and on-call rotations.

Required Qualifications

• Vocational/Technical Training or Associate Degree – Registered Nurse
• 1 year Long Term Care
• Registered Nurse with the MN Board of Nursing
• Basic Life Support (American Heart Assoc, Red Cross or MTN)
• AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor. American Red Cross: CPR/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
