Job summary

PACS is seeking a Nursing Supervisor to oversee the facility’s resident assessment process, including MDS and PDPM activities. You will lead weekly PDPM meetings, monitor tool utilization, and provide data-driven recommendations to improve outcomes.

The role requires a BSN and state licensure, with 3 years of supervisory experience and LTC familiarity. Strong MS Office and PCC experience are preferred.

Qualifications

• Accredited School of Nursing with a BSN preferred.
• Current state nursing license and continuing education requirements.
• At least 3 years supervisory experience; long-term care experience preferred.
• Proficient in Microsoft Office; PCC knowledge preferred.

Responsibilities

• Oversight of facility weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools and performance metrics.
• Provide PDPM and MDS training to new coordinators.
• Assess health status and care level for new admissions.
• Ensure timely completion of all MDS assessments and data submission.
• Coordinate MDS with interdisciplinary teams within regulatory timeframes.
• Schedule and conduct resident care conferences; revise care plans as needed.
• Maintain confidentiality of resident care information.
