Behavioral Health Network, Inc. is seeking a Nurse Care Manager to join its One Care team in Western Massachusetts. The role focuses on advocacy, care coordination, and empowering individuals to participate in their care.

Responsibilities include coordinating with primary care and behavioral health providers, documenting progress, and supporting transitions from inpatient settings to the community. A RN license and relevant experience are required.

Qualifications

• RN license required.
• High School Diploma or GED required.
• 1-2 years of related behavioral health or medical field experience preferred.
• Cultural and linguistic competence with diverse populations.

Responsibilities

• Advocate for individuals served and empower informed decisions about care.
• Involve integrated care team including primary care and behavioral health providers.
• Maintain relationships with community partners and coordinate referrals.
• Document interactions and outcomes; prepare reports for stakeholders.
• Promote transitions from higher levels of care back to the community.
