Job summary

Behavioral Health Network, Inc. is seeking a Nurse Care Manager to join our CBHC Care Management team in Western Massachusetts. The role emphasizes advocacy, service coordination, and safety monitoring to improve client outcomes.

You will secure services, maintain documentation, and collaborate with community partners to ensure high-quality, person-centered care. Clinical and field outreach are integral to building stable community supports and reducing unnecessary hospital utilization.

Qualifications

• High School Diploma or GED required.
• Registered Nurse license required.
• 1–2 years in behavioral health, human services, and/or medical field required.
• Cultural and linguistic competence with diverse populations.

Responsibilities

• Advocate for individuals served, supporting autonomy and self-determination.
• Secure appropriate services and monitor progress for quality care.
• Identify safety issues and ensure well-being in consultation with supervisor and leadership.
• Build and maintain relationships with community partners and providers.
• Document interactions, progress, and referrals for funding and stakeholders.
• Reduce higher levels of care when appropriate and assist transitions back to community.
• Review MDS assessments to identify health needs and gaps in care.
• Implement person-centered care plans in collaboration with individuals and coordinators.
• Provide care management including post-hospital follow-up and medication reconciliation.
• Outreach to individuals served in home, office, or community to address health needs.
