Job summary

CareSource/Commonwealth Care Alliance in Massachusetts seeks an Integrated Care Clinical Manager (RN) to lead complex care management for dually-eligible enrollees in community settings. The role centers on coordinating medical, behavioral, and social services, driving care plans, and collaborating with an interdisciplinary team across the continuum of care with regular travel within the state.

Qualifications

ASN in nursing from an accredited program.
Current MA RN license to practice.
Case Management Certification preferred.
Valid driver’s license and vehicle.
Experience with dual-eligible populations preferred.

Responsibilities

Engage enrollees in homes and community settings for complex care management.
Serve as liaison between providers, resources, and enrollees to ensure seamless care transitions.
Perform Comprehensive Assessment and functional assessments including MDS-HC.
Lead interdisciplinary care team to develop holistic care plans addressing medical and non-medical needs.
Oversee enrollee utilization of long-term services and supports and coordinate access to resources.
