Job summary

Behavioral Health Network (BHN) is seeking a Care Coordinator to join the One Care team and help empower individuals to live healthier, more fulfilling lives in Western Massachusetts.

The role involves providing telephonic, virtual, and in-person support, coordinating housing, healthcare, and community resources, and maintaining collaborative Integrated Care Teams to ensure quality, person-centered care.

Qualifications

• Must have strong interpersonal skills and empathy.
• Ability to navigate complex systems and coordinate services.
• Experience with behavioral health or medical field preferred.

Responsibilities

• Provide telephonic, virtual, and in-person support and guidance to individuals served.
• Identify individuals in need and engage them to connect with resources.
• Advocate for individuals to promote autonomy and informed care decisions.

Skills

Bilingual Spanish/English

1–2 years experience

Cultural competence

Effective communication

EHR familiarity

Driver's license

Education

High School Diploma or GED

CHW Certification

Bachelor's degree preferred
