Job summary

CareSource/Commonwealth Care Alliance in Massachusetts seeks an Integrated Care Clinical Manager to oversee complex care management for dually-eligible enrollees. The RN will coordinate medical, behavioral, and social services, building holistic care plans and guiding an interdisciplinary team.

The role requires an MA RN license, care coordination experience, and strong communication. Frequent community travel is expected; a robust total rewards package and performance bonuses are offered.

Qualifications

• AS in Nursing from an accredited program.
• Current RN license in Massachusetts.
• Case Management Certification preferred.

Responsibilities

• Engage enrollees in home and community settings to build complex care relationships.
• Lead ICT to develop holistic care plans addressing medical and social needs.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Monitor enrollee utilization of long-term services and supports.
• Educate enrollees about benefits and preventive care.
