Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts to oversee complex care management for dually eligible enrollees. This field role requires RN licensure, strong collaboration with interdisciplinary teams, and liaison with providers and community resources to coordinate care.

You will lead care planning, assess needs, and promote adherence to Medicaid/Medicare benefits while ensuring high-quality outcomes and patient satisfaction.

Qualifications

• ASN degree from an accredited nursing program.
• RN licensed in MA and able to independently serve dual-eligible populations.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage enrollees in care in homes and community settings, considering diverse needs.
• Lead interdisciplinary care team to develop holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless care and avoid duplication.
• Monitor enrollee utilization of long-term supports and ensure access to resources.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
