Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts to oversee community-based RN care management for dually-eligible enrollees with complex needs. The role focuses on integrating health services with community resources to improve outcomes and quality of life.

The position requires an RN with an ASA in Nursing, licensure, and experience in care coordination or dual-eligible populations. Strong communication and field leadership are essential for success.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• RN license to practice independently with complex care needs.
• Care coordination or dual-eligible population experience preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical Field/Community Based Training a plus.

Responsibilities

• Engage enrollees in their homes and communities to build a complex care management relationship.
• Serve as liaison between healthcare providers, resources, and enrollees for care transitions.
• Perform comprehensive assessments (e.g., MDS-HC) and risk assessments.
• Lead the interdisciplinary care team to create holistic care plans.
• Oversee enrollee use of long-term services to support preference for staying at home.
• Assist members in accessing housing, transportation, and other social services.
• Educate members about Medicare/Medicaid benefits and services.
• Promote self-management and preventive care strategies.
• Follow up after hospitalizations to ensure continuity of care.
