Job summary

Qualifications

• RN licensure in Massachusetts is required.
• Experience in care coordination or dual-eligible populations preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Lead the interdisciplinary care team to develop holistic care plans.
• Coordinate with providers, social services, and community resources.
