Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager (RN) to monitor, follow up and manage care for dually eligible enrollees with complex medical, behavioral, and social needs. The role integrates health services with community resources to improve outcomes and quality of life.

The position is mobile, requires regular home visits and travel within the Commonwealth; you will lead the interdisciplinary care team, coordinate with providers and community partners, and support

Qualifications

• Associates of Science in nursing from an accredited program.
• Certified RN with MA license to practice independently.
• Experience in care coordination or dual-eligible populations preferred.

Responsibilities

• Engage enrollees in home and community settings to build care plans.
• Lead interdisciplinary care team to develop holistic plans.
• Coordinate with providers and community resources for seamless care.
• Perform assessments and monitor chronic conditions and preventive care.
• Ensure access to housing, transportation, food and social services.
