Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts to coordinate care for dually-eligible patients with complex needs. The RN will manage assessments, care plans, and transitions while partnering with providers and community resources to improve outcomes.

The role requires MA RN licensure, mobility within the territory, and strong communication and teamwork skills to lead an interdisciplinary team and ensure high-quality, person-centered care.

Qualifications

• Current unrestricted RN license in Massachusetts required.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare knowledge helpful.

Responsibilities

• Provide in-home and community-based clinical care management for dually-eligible enrollees.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with healthcare providers and community resources to avoid duplications.
