Job summary

Hackensack Meridian Health Inc. seeks an Advanced Practice Nurse to deliver patient-centered, coordinated care in collaboration with the hospital team.

You will assess, plan, and implement care for patients with health and safety needs, while maintaining clinical competencies and leadership in practice. The role requires NP licensure in New Jersey, prescriptive authority, an NPI, and a collaborative physician agreement.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Responsibilities

• Leads direct clinical care and collaborates with the care team.
• Provides coaching and guidance to staff and patients as needed.

The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Education, Knowledge, Skills and Abilities Required
