Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse (APN) on a per diem basis to provide patient-centered, coordinated care.

You will work with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The role emphasizes acute clinical management, performing intake assessments, and reviewing EHR to determine eligibility for program inclusion and safe home transfer.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Responsibilities

• Review extensive EHR to determine if patients meet criteria for inclusion and transfer home.
• Conduct intake assessments to determine if a patient's acuity and home environment are safe for the program.
• Weekend Work - Weekends as Needed.

Job description

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

This per diem position will require APN acute clinical management experience. Extensive review of EHR to determine if the patient meets criteria for inclusion and transfer home. Conduct in-take assessments to determine if a patient's acuity and home environment are safe for the program.

Weekend Work- Weekends as Needed
