Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse (APN) to deliver patient-centered coordinated care, lead and collaborate with the practice/hospital team in assessing, planning, and implementing care for individuals with health and safety needs.

The role requires NP licensure in NJ, prescriptive authority, and board certification, with opportunities for ongoing professional development and cross-disciplinary collaboration to improve patient outcomes.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate in NP program.
• Maintain clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Job description

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Education, Knowledge, Skills and Abilities Required:

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
