Job summary

Hackensack Meridian Health Inc. seeks an Advanced Practice Nurse (APN) to lead patient-centered care within a transition team. The APN will collaborate with physicians and hospital staff to assess, plan, and implement comprehensive care for patients with complex health needs.

The role requires NP/Doctorate or MSN with APN certification, NJ RN and APN licenses, NPI, and prescriptive authority per state law. Strong communication and IT skills are essential, and weekend/holiday coverage is

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office and/or Google Suite.

Job description

The Transition Team Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Weekend WorK: every Fourth Weekend
Holiday Work: Two of 6
