Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse (APN) to provide patient-centered, coordinated care within the practice/hospital team, focusing on leadership, direct clinical practice, consultation, coaching, and research.

This role requires an NLN/AACN accredited nursing program graduate, NP master's or doctoral degree, state nurse practitioner licensure, prescriptive authority, and board certification (AANP/ANCC).

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Education, Knowledge, Skills and Abilities Required:
