Job summary

Hackensack Meridian Health Inc. seeks a Palliative Care Advanced Practice Nurse (APN) to deliver patient-centered, coordinated care within a multidisciplinary team. The APN will lead clinical practice, participate in consultations, coaching, and research, and apply ethical decision-making to complex cases.

The role emphasizes prescriptive authority under NJ law, collaboration with physicians, strong communication, and quality patient care in a hospital-based palliative setting.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Job description

The Palliative Care Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Education, Knowledge, Skills and Abilities Required:
