Job summary

OptumCare is seeking a nurse practitioner or physician assistant to provide patient assessments in nursing homes, senior housing, and assisted living facilities as part of the Senior Community Care (SCC) team. You will document findings, coordinate care, and communicate with attending staff during transitions from rehab to home.

Responsibilities include thorough medicational reviews, CMS-compliant documentation, and regular travel to sites within the designated area.

Qualifications

• NPs: MSN and board certification (AANP/ANCC) or equivalent.
• PAs: NCCPA certification.
• Active, unrestricted license in resident state.
• Ability to move a 30-lb bag and navigate stairs.
• Available to work 10 hours per week.
• Driver's license and reliable transportation for travel to sites.

Responsibilities

• Perform comprehensive assessments and document findings per CMS requirements.
• Ensure diagnoses are ICD-10 coded and documented.
• Confirm quality elements are addressed and documented.
• Conduct initial medication review.
• Attend mandatory trainings and complete required trainings.
• Travel between care sites as required.
• Coordinate care and communicate with patients and team members.
