Job summary

As an Advanced Practice Clinician (NP or PA) in the Senior Community Care (SCC) program, you will provide comprehensive clinical services to high-risk patients, improving outcomes and quality of life.

Qualifications

• Current ANCC or AANP certification in family, adult, geriatric, or adult-gerontology primary care.
• Current NCCPA certification for Physician Assistants.
• Active, unrestricted state license and driver’s license.
• Current DEA license or ability to obtain after hire, per state requirements.
• Ability to establish a collaborative practice agreement if required by state law.
• Ability to perform job duties across various patient care environments.

Responsibilities

• Conduct comprehensive patient assessments including history, medications, physical exams and vital signs.
• Support members during short-term skilled nursing and rehabilitation stays.
• Ensure timely and accurate clinical documentation including ICD-10 coding.
• Collaborate closely with physicians, nurses, care coordinators and other healthcare professionals.
• Educate and counsel patients and caregivers on health conditions and care plans.
• Facilitate advance care planning and discuss treatment goals.
