Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care (SCC) team. You will assess high-risk patients in nursing homes, senior housing and home care settings, coordinate care, and drive advanced care planning.

No on-call, weekends, or holidays required. You will work with an interdisciplinary team to deliver comprehensive care, ensure thorough documentation, and support patients and families through transitions.

Qualifications

• Active ANCC or AANP certification in family/geriatric primary care.
• NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain post‑hire.

Responsibilities

• Conduct comprehensive assessments including medical history, vitals, exam and medication reconciliation.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation including ICD-10, care plans and progression notes.
• Collaborate with interdisciplinary care team and communicate effectively.
• Lead advanced illness and end-of-life planning conversations with patients and caregivers.
• Provide counseling and education to patients and families.

Education

ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care

NCCPA certification and state licensure

Active, unrestricted state licensure

DEA licensure/prescriptive authority

Collaborative practice agreement eligibility

Driver's license
