Job summary

Optum Home & Community Care, part of the Optum family, seeks an Advanced Practice Clinician (NP or PA) to deliver coordinated, patient-centered care to health plan members in SCC programs serving older adults in nursing homes and senior housing. You will improve outcomes and quality of life through comprehensive clinical services.

Qualifications

• Must hold ANCC or AANP certification in Family/Adult/Geriatric/Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and applicable state licensure.
• Active, unrestricted state license and driver's license.
• DEA license or ability to obtain after hire.
• Ability to establish a practice agreement per state law.
• Experience in long-term care or geriatrics preferred.

Responsibilities

• Conduct comprehensive patient assessments, including medical history reviews, medication reconciliation, physical examinations and vital sign assessments.
• Support members during short-term skilled nursing and rehabilitation stays.
• Ensure timely and accurate clinical documentation, including ICD-10 diagnosis coding.
• Collaborate closely with physicians, nurses, care coordinators, and other healthcare professionals.
• Facilitate advance care planning and discussions regarding treatment goals; educate patients and caregivers on health conditions and care plans.
