OptumCare is seeking a Nurse Practitioner or Physician Assistant for the Senior Community Care team in Cumberland County. Qualifications

• NP or PA board-certified with required degree.
• Active and unrestricted license in the state you reside.
• Ability to lift 30 pounds and navigate stairs/varied dwelling conditions.
• Available to work 10 hours per week.
• Driver's license and reliable transportation for travel to client sites.

Responsabilités

• Perform comprehensive assessments and document findings per CMS requirements.
• Ensure diagnoses are ICD-10 coded and documented.
• Conduct initial medication review and care coordination.
• Attend mandatory trainings and travel between care sites.
